Waiver & Forms

Participation Waiver
This waiver is required to be filled out and turned in to participate in any Umpqua Valley Gymnastics program, Birthday party, Camp, Event, Etc. If you are already registered for a normal class it is not necessary to fill out again.
Birthday Parties!
Please print, complete and turn in your birthday party form to the the front office for approval of your request.
Private Lessons
Please print, complete, and return this form to your coach with $5 attached. You will pay your private lesson fee directly to your coach.
Scholarship Application
Please print, complete, and return the form to the front office.  A scholarship committee will review all applications, to review and determine any awards that are given based on need.
