This waiver is required to be filled out and turned in to participate in any Umpqua Valley Gymnastics program, Birthday party, Camp, Event, Etc. If you are already registered for a normal class it is not necessary to fill out again.
Please print, complete, and return the form to the front office. A scholarship committee will review all applications, to review and determine any awards that are given based on need.
Please fill out and submit if you are interested in joining our program! No experience needed, you just need to love working with children and we will train you!
